DANVILLE - There is an unusually high level of animosity within the community following Danville Regional Medical Center’s transition from public to private ownership, health care consultant Keith Pryor said Monday.
“Your situation bothers me,” said Pryor, who was hired to work with the Citizen’s Commission Related to the Danville Regional Medical Center. “What shocks me is the extremely high level of discord that you are having here.”
Pryor met with the commission Monday to discuss the quality of care at Danville Regional.
He said the biggest problem facing the hospital is a growing lack of trust between members of the community and LifePoint Hospitals Inc. Pryor said this problem started when LifePoint purchased Danville Regional in July 2005.
Pryor said the transition from public to private ownership requires “leaders who are the best of the best” and that LifePoint’s own admission that it made leadership mistakes during this time did not make the process any smoother.
“There is a lot at stake here,” he said, adding the level of distrust could hinder anything from the community’s health care system to its economic development efforts. “(This problem) is so unnecessary and I think you can fix it.”
Pryor told the commission members the best way to restore the trust was to open up a dialogue between LifePoint officials, community leaders and physicians at the hospital.
The participants in this dialogue must be willing to leave the past behind them and focus on some of the core problems that were identified through the commission’s efforts, he said.
“It’s time to start walking on a new path,” said Jim Houser, co-chairman of the commission, adding he felt the “old path was very negative and destructive.”
Houser said the commission identified five areas the hospital needed to improve at its last meeting, including staffing levels, poor patient care, wait times in the emergency room, morale among employees and administrative issues.
Houser said these topics covered most of the problems that were brought up at a recent series of public hearings, but some positive things were said about the hospital as well. People testified that the hospital’s cleanliness, one-day surgery program and heart center seemed to show improvement.
Pryor said some other “good news” was that about 80-90 percent of the community and hospital staff wanted the situation at Danville Regional to improve.
He said LifePoint has no other choice but to listen to concerns because Danville Regional’s situation may hinder the company’s ability to grow.
“We too have come to the same conclusion. We have to build trust,” Danville Regional CEO Art Doloresco said Monday at the commission meeting. “Building that trust means having the dialogue that (Pryor) talked about.”

Why on earth couldn't Art and Lifepoint have drawn these same conclusions without the help of 25,000 of our dollars and numerous hours by the commission and citizens?

Hint: Next hospital you buy, It'll be a lot easier to MAINTAIN trust than ever will be to REBUILD it.
Art will say whatever he needs to, to save his job.
An initial step toward the path for recovery at DRMC would be the removal of Doloresco from his current position as CEO. Why...not because Art is a villain, as portrayed by many but as a result of less than stellar decisions made autonomously during his tenure in Danville.
Tactics such as delaying and "spinning" his respinses to innocent inquiries from employees of DRMC and citizens of the community have ultimately resulted in a loss of professional credibility. Although intelligent and touted as experienced, I'm concerned that Art lacks the ability to "tune in" to the culture of the organization or the community. Without that capability, how might he identify the extraordinary needs necessary for the recovery of DRMC?
Therefore, if LPNT has the faith they profess in Doloresco's leadership as a CEO...relieve him from DRMC and "gingerly" place him as CEO in another facility where his management style might be more congruent with it's culture.
A good point was made in a comment to this story on the registerbee website:
"Hopefully Mr. Deloresco will, (if he has not already done so), place the hospital's "Patient Advocate" function under a "Quality Improvement Section" reporting to him, rather than have the patient advocate under "marketing". A restructuring move like this would give the community and staff a boost in confidence, monitor and improve care. This also raises the bar for patient advocacy to assure that future satisfaction surveys are valid care assessments and do more than just "improve numbers"!"
Why "Quality Improvement Section"?? Isn't that another part of DRMC poor leadership who has just recently been asked to step down??
CMO announced resignation today...will remain "education director". Rumor is new "O" (didn't say which "O") will be in place next month...
So the expert is "bothered" by what's going on at DRMC. I think that shows he did nothing to try to understand the agony and horror of some of the experiences.
Of course not. Look who wrote him a check for $25,000. I went to the City Council meeting and they sat right there and approved the payment without a dissenting vote.
So what do you expect?
Dialogue, schmialogue. Even an 8th-grader should get an "F" for that vacuous a conclusion on a report. Bottom line: Pryor briefly showed up, and collected $25,000. Talk about "low hanging fruit" ! Danville: suckered again!

Rescue squads, 911 operators and hospital emergency departments all over the country struggle with non-emergency emergencies. People who don’t have real emergencies overload the system.
Here in Danville, waiting times in Danville Regional Medical Center’s Emergency Department have been an issue for years. In response to the problem, the hospital recently opened a new urgent care clinic.
The hope is that people who realize they don’t have a life-threatening emergency will go to the clinic instead of the Emergency Department.
“This is for if you have strep throat or a virus, or you’ve just been sick for a couple of days,” said Cathy Shelton, director of Danville Physician Practices. “… True emergencies are life threatening. If you come in with chest pains or anything life threatening, we will send you into the (Emergency Department).”
Shelton said the urgent care clinic hopes to treat 10,000 people per year.
One test for this clinic - and a second one Danville Regional hopes to open in Brosville - will be its effect on waiting times and non-emergencies in the Emergency Department. In all fairness, though, another test will be to see if hospital patients are able to use the urgent care clinic correctly.
If that happens, a big load will be taken off the system - and people will face shorter waits for treatment.
Even this clinic has its limits, though. It will be open from 5 to 9 p.m. Monday through Friday and from 1 to 9 p.m. Saturday and Sunday. That’s just 36 hours per week. While those hours could change, it still leaves more than 100 hours per week for people to make the right choices about the kind of medical care that fits their needs.
The track record - both in Danville and across the country - isn’t good. And those poor choices have helped to push up the cost of health care in this country because hospital emergency departments are expensive to equip and staff.
Danville Regional’s new urgent care clinic is one of many solutions to solve the problem of overcrowded hospital emergency departments. Let’s hope it works here.

The wait time problem will (continue to) self-correct. As more and more commercially insured patients choose to go to other hospitals, and as more and more doctors' offices offer ambulatory care, the wait times will continue to go down. This would have been a great solution a couple years ago when there really was a need for a 'fast track', non-emergency option for the ER.
I mean, even the Caswell EMS folks aren't driving north any more, right?
Jess Judy is signed on for four weeks in Danville.

I'm an ICU nurse who currently works for Moses Cone along with 5 other nurse none of whom have been asked to return to Danville. And as far as I know, none want to return!!

June 12, 2007 (KABC-TV) - Edith Rodriguez, a 43-year old mother of three, died last month, writhing in pain on the floor of King Harbor Hospital's emergency room lobby, as the staff at the hospital allegedly ignored her.
On Tuesday, recordings of 911 calls were released. On the calls, other people in the emergency room tried in vain to get help.
It is a case sending shockwaves throughout county government and the community that the hospital serves.
The hospital, King/Harbor (formerly King-Drew) Medical Center, has been through numerous overhauls, failing time and again to make necessary improvements.
The death of 43-year-old Edith Rodriguez displays a serious lapse, according to the supervisors.
She was discharged from the hospital, yet she remained in serious pain. She never left the hospital grounds; she got to the emergency room and collapsed.
Recordings of two 911 calls were released on Tuesday. One is from a bystander, the other is from Rodriguez's boyfriend, Jose Prado, who spoke through a interpretor and tried in vain to get help.
Operator: I'm trying to get an interpreter for you, sir ... can you find out what his emergency is, please?
Caller (Prado through an interpretor): I'm in the emergency room, my wife is dying. The nurses don't want to help her out.
Operator: OK, what do you mean she's dying? What's wrong with her?
Caller (Prado through an interpretor): She's vomiting blood.
Operator: OK, and why aren't they helping her?
Caller (Prado through an interpretor): They're watching her. They're watching her there and they're not doing anything. They're just watching her.
A bystander also tried to get help for Rodriguez.
Operator: What's your emergency?
Caller: There's a lady on the ground in here, in the emergency room at Martin Luther King and they are overlooking her, claiming that she's been discharged and she's definitely sick, and ... they're just ignoring her.
Operator: What do you want me to do for you, ma'am?
Caller: Send an ambulance out here to take her somewhere where she can get medical help.
Operator: OK, you're at the hospital ma'am. You have to contact them.
Caller: They have a problem, they won't help her.
Operator: Well, you know if you're not pleased with the result you're getting from them, you know we can't ...
Caller: Well it's another patient. I'm not pleased with the results.
Operator: 911 is used for emergency purposes only, life-threatening emergencies.
Caller: This is an emergency.
Operator: It is not.
In addition to the 911 tape, the supervisors have viewed surveillance video of the emergency room. They are appalled at how many people ignored Rodriguez.
Much of the blame was placed on the head nurse who failed to assess the gravity of the situation.
"Here you had a situation where you had a dozen people in a emergency room, nobody got up to help. The janitors came over to clean the vomit around the victim. They did a diligent job cleaning the vomit but they didn't take one look at her," Zev Yaroslavsky, L.A. County Supervisor, said.
"If you have the wrong people who are in charge, you cannot turn around something that starts going in a very negative way. This nurse who made the decision that this woman was not really hurt that badly or that she was, I don't know what her decision was, but she obviously decided the woman was not in the need of immediate care. That should have been turned around," Yvonne Brathwaite-Burke, L.A. County Supervisor, said.
King/Harbor Medical Center has been under fire since 2003 because of numerous deaths under questionable circumstances.
Scores of people have been fired, including the head nurse who was involved in that latest incident. The hospital remains under federal investigation.
The board of supervisors is calling for more training, but it has been a challenge. Supervisor Yaroslavsky: "How do you train a person to care?"

CFMC seeks help in filling local need

Jun 13, 2007 - 12:00:14 am CDT Shirley Deal, Executive Director of Caswell Family Medical Center, says the CFMC board recognizes the need for emergency healthcare in the county, but stresses that it will take the support of the entire community to enable the community health center to help fill that need.
Established in 1978, CFMC is a non-profit organization governed by a board of directors comprised of people who use the center's healthcare services. Fifty-one percent of the board must be people who use CFMC for their healthcare.
Twenty-five percent of the center's operating expenses are funded by federal grants while the rest is covered by operating revenues generated by patients with third party insurance. Thirty-six percent of their patient population has private or corporate health insurance (other than Medicare or Medicaid) but CFMC also uses a sliding scale fee to ensure that everyone in Caswell County can receive quality healthcare, regardless of their ability to pay or healthcare coverage.
"We could be the medical center that this community really needs," Deal said. "But the federal grants do not keep up with the rising cost of healthcare. If we're going to be the place that people want, it's going to take their help."
As the county's EMS service continues to juggle limited resources to efficiently handle a rapidly increasing call volume, transport times continues to be a major concern. Every time an ambulance transports a patient, that ambulance is being taken out of the county to hospitals in Danville, Alamance, Rockingham, Greensboro, Duke and Chapel Hill, depending upon the level of care the patient requires. EMS has recently made changes to their staging locations in an effort to improve response times, but with no hospital or emergency facility in the county, there is nothing that can be done about the hour or two an ambulance is out of the county transporting a patient.
It has frequently been suggested by various local leaders that CFMC begin offering a certain level of emergency care services with extended hours so that county residents would not have to go out of the county to receive basic emergency care.
Deal says that the CFMC recognizes that need in the community and wants to do what they can to fill that need, stressing that it is a matter of limited funds and limited space.
Deal added that CFMC has several specialists willing to serve the county's population, eliminating the need for many out of town trips for healthcare, but added that the center is currently busting at the seams. There is simply no room in the current facility to increase the number of providers at the moment.
Currently, CFMC provides primary medical care, diagnostic laboratory and radiological services and preventative services such as well child services, immunizations, screening for elevated blood lead levels, communicable diseases and eye, ear and dental screenings for children. They have three doctors and two mid-levels on staff, with one of the doctors being double board certified in pediatrics and internal medicine. These providers provide healthcare to 17,000 patients a year.
Deal says that expansion is necessary to allow the center to grow to meet the ever-growing healthcare needs of the county, but emphasizes that such growth will not be possible without the financial support of the community.
The center currently has no private benefactors, a fact the board hopes will change soon.
"There are many people who have roots here in Caswell County and have the means and desire to give back to the community," Deal said. "I hope they would consider the possibilities of leaving a continued legacy by supporting CFMC to continue to provide improvements to our facility."

Just spoke with someone who said "managers were handed a letter stating that after their second meeting, joint comm. upheld the preliminary denial of accredidation, to pass that information on to all employees and not to ask any questions. It is what it is"

Notice there haven't been any since LFPT
It seems DRMC is not the only hospital having issues with inspections. Seems a VA hospital in Seattle also has issues with Joint Commission standards. I am not excusing DRMC. We all need to be aware that accrediting agencies frequently raise the bar in all walks of life. The rules today are more stringent than they were a hundred years or so ago. Look at how factories, the food industry, and others operated and the working conditions. Very different. We have grown as a society and continue to grow. Does this mean we should not expect the best at DRMC? Of course we should. This is our lives we are talking about! We just need to be aware that the failures may include something as simple as a committee not meeting every month or a very busy nurse not knowing the answer to a question right off the top of their head. The administration needs to stop and look at where we were pre-Lifepoint, where we are now and where it seems we are heading. An overhaul seems in order. Art and company need to look beyond the "snowballers" and "yes-men" on Leadership Council and their own personal agendas. Listen to the people in the trenches doing the work day-to-day. We know what is wrong and may be able to provide solutions to fix them. A multi-departmental group including middle management and others who actually do the work would be helpful to discuss what problems we have with each other and together resolve them. No one person knows everything about all the departments. We have to work together to come out of this. Mandates from Administration alone will not fix this. Art, are you CEO enough to do this right? We are waiting.
So, what was it that they couldn't fix? At the time of the first denial, Mr. Doloresco said it was none of the public's business.
Is it our business this time?
Or will LifePoint's arrogance continue to dominate?
So after all that, they failed again. I suspect the situation is hopeless. Can someone tell me of any reason for hope at this point?

In response to one of the comments. I'm an X-DRMC ICU RN who left for UNC with 5 others. Only 1 returned and that was due to scheduling conflicts and the 1 hr drive to UNC.
DRMC did not fail again. The preliminary denial was upheld. Read between the lines people...it is no longer preliminary.

DANVILLE - The Danville Regional Medical Center is the only hospital in Virginia to currently have a “preliminary denial of accreditation” status from the Joint Commission. Joint Commission spokeswoman Elizabeth Zhani answered a few questions about what this means Monday afternoon.
Question: What is the Joint Commission?
Answer: The Joint Commission is an independent not-for-profit organization that monitors care given at nearly 15,000 health care organizations in the United States. The group evaluates care at 97 hospitals in Virginia.
Q: What is a preliminary denial of accreditation?
A: A “preliminary denial of accreditation” status is one step above a full denial of accreditation. The commission gives this status to health care organizations it feels justified in denying its accreditation to, but it lets the organization appeal this ruling before the actual denial takes place.
Zhani said large hospitals like Danville Regional that fail to meet 17 or more standards of care during an evaluation are given this status. She said approximately 1 percent of the hospitals surveyed across the U.S. in 2006 received a “preliminary denial of accreditation.”
Q: When did all of this happen?
A: The Joint Commission sends surveyors to evaluate a hospital once every three years. It has sent teams to 23 Virginia hospitals since June 2006. Commission surveyors last visited Danville Regional on Feb. 13-16.
Danville Regional CEO Art Doloresco sent out a news release in March letting people know the hospital may receive a “preliminary denial of accreditation” following this visit. Zhani said the commission’s accreditation board reviewed the survey findings last week and signed off on the decision.
Q: What happens next?
A: Zhani said the hospital could appeal its “preliminary denial of accreditation” status to the commission’s review panel. She said the panel will look over the original survey results and “can deny the accreditation or make another decision” about the hospital’s status. Zhani expects this hearing to take place within the next month. No hospital official would return calls Monday for comment about their plans for the future.
Q: Why is this important?
A: State and federal governments use Joint Commission evaluations as a way to determine whether a health care organization qualifies for Medicare and Medicaid reimbursements. If a hospital loses its accreditation, the government will have to send its own team of investigators to evaluate the hospital before any decision becomes final.
